So your router is NATing your repro box then.  Your repro transports need to be bound to your local interface and you need to use the public IP in any Record-Route configuration.  You will want to turn on force record routing as well.  Forwarding the single SIP port is enough for SIP traffic.

I'm fairly certain that reTurn will NOT work behind a NAT device.  I recall that someone had toyed with some settings for that but I don't think it was ever put in the release.
