[reSIProcate] can resip provide refer with replace callid, not invitesessionhandle?

If you don't want to modify resip code to do this, you can try just
passing in your own InviteSessionHandle and modifying the call id and
to/from tags in the replaces header of the refer message using the
onReadyToSend callback.
