[reSIProcate] DUM for sippy/gag

The design of DUM is getting pretty close, but there is a lot of
implementation left.  The InviteSession stuff will be in alpha soon, and I
plan to re-work some of the lifetime management, but that should be done
soon.  I'll ping the list when I get the basic test running, there are a lot
of corner cases with assert(0)'s that need to be resolved could use another
pair of eyes, and some it is probably wrong.

Server and Client subscriptions are sashimi grade, and I need them soon for
call transfer, but I'd like to talk about how they interact with the
InviteSession usage in that case.
