For NAT traversal - RFC5626 (Outbound) recommends you register to both a primary and back up registrar.  

Outside of outbound I'm not sure off hand what the best / recommended solution for your problem would be.  I would be interested in hearing from others how they solved this problem.

two ip addresses, this is for load balancing. We've noticed that incoming calls
can sometimes be originated from the secondary server whilst the regisration 
was done to the primary server. The UDP keepalives are only sent to the primary
servers so incoming INVITES from the secondary server are dropped by the
firewall. 

Should resip be sending UDP keepalives to both proxy servers which are resolved
by DNS SRV, I think its only sending them to one. Or is there some other way we
should be handling this?
